The tractor would have been cool but you never get to drive the weird and neat vehicles in games

Not sure if you've played on from the end of this, but yes, you're infected. When that little skull timer gets down to 0, you'll start slowly losing health until you rid yourself of the infection. You can use rags and painkillers to keep your health up, but you'll keep losing health until you're cured. You have to craft antidotes at the chem workstation (in your build menu) and it will take multiple shots of the antidote. They purposefully designed the game so that the infection persists after death / respawn. I imagine that was to prevent players from simply respawning to rid themselves of an infection. They did put an option in the settings to turn the persistent infection mechanic off because of player feedback.
