January 4, 2024: NTSB Chair Jennifer Homendy briefs the media on the final report of the NTSB investigation of the 2021 WMATA train derailment in Arlington, VA.

This could have been much worse. I've ridden that line. As a passenger riding that train near where the car derailed would I know there is a derailed car that had re-railed itself? And if I knew that would there be a way to notify the train operator? What if it jumps the track at 50 mph? I think I might want to get off that train at the next stop.
Excellent! Finally, someone to stand up to this cabal: the Freight Carrier Railroad Engineering FRA Revolving Door Cabal. The problem with writing regulations though is where we'll probably be getting into trouble. Railroads used to write their own dimensions, minimum radii, etc. The FTA has already come out with some funny ones. But you see the fiduciaries and regulators don't even know it! These are no longer the heady days when the 1st segments of Washington Metro were coming into being. BART too, despite the gauge thing – those were our two last great rail construction projects. They don't build stuff like that anymore unless it's deeply flawed in such a way as to make it minimally useful: See CAHSR, HS2, and a few other things. This is what she's fighting against, though she may not even know it. I wish her success.

Hi guys…
Isn't this just another example of incompetence.
Why are wheelsets held together with only friction? Can't they be welded???
Thank you for sharing the information, findings, and reccomendations. The NTSB does a great job!
I love that she does not pull punches. As a federal LEO, I would love to see her as head of DOJ.
Wow she is *pissed*. And rightfully so. WAMATA "didn't know" because they didn't want to know.
the NTSB is based

I've always thought assembling wheelsets with a press fit was a dumb idea.
