Narratively, they make a great enemy for imperial forces to stamp out and you can play them in Necromunda, Killteam, and 40k. There are even rules out there for using them in Space Hulk.

Most people house rule that regardless of how many familiars a model has, they can only do Omen of fortune once. Otherwise it gets really unbalanced
