Here's a suggestion: Fallout 4 without healing radiation. No RadAway, no visiting a doctor, no decontamination arches. Increasing Rad Resistance is fine, but whatever radiation you get is with you for the rest of the run. And since the BoS and Institute routes have a forced decontamination arch during Mass Fusion, you can't do either of those.
