It's almost always the multi billionaires that are at fault. Cutting costs , hiring too few people, cramming as many rides or people into the business. And those multi billionaires lost no sleep other than that it cost them money.
I have heard about this accident. Feel bad for the pilot's family. Sadly there was another monorail accident in Disneyland in 1966 when someone sneaking into the park got run over by the train but in that case, was purely the fault of the victim.

Unfortunately, disney has had multiple issues with protocols that surround accidents. There was the roger rabbit taxi ride accident that trapped, disabled, and ultimately killed a young boy where he was trapped under a ride vehicle, but the protocols delayed him getting help that could have saved him. There was also the accident at Disneyland with the big thunder mountain railway, where someone was killed because the ride wasn't taken out of service before it failed, even though it should have been. I am so tired of seeing that same hand being wrapped that has nothing to do with the story
Another thing that happened is that as a guest (3 in total could ride in the front with the pilot) Not so anymore. As a former ride op. I was both conductor + engineer at a themed park. You were always supposed to "guard the rear" of the train when reversing. At least 3 pairs of eyes. Conductor, supervisor/mechanic at the switch + the engineer or fireman. Accident should have NEVER happened. You could ride front/rear of Disneyland's. The windshields are so aerodynamic they need no wipers. Another design flaw was that being fiberglass/aluminum those front ends are as flimsy as paper mache' in a collision.
Before this accident you could ride in the front car of the monorail with the pilot. They even gave the little ones a button that said "official monorail co-pilot."
They used to allow guests to sit up front with the pilot. My family got to do that. After this tragedy that was no longer allowed.
