I mean, in the game, it's said by the resident expert in game that all this stuff is encoded to happen in our DNA, ready to set off in about 200 years or whatever. All he did was release a retrovirus or whatever you call it to alter the dna to set it off early in an effort to get enough people who are 'immune' or rather, people who live through whatever the mutations it's trying to pull off with their minds intact so he could combine it with the blood of his designer baby daughter to make a serum that essentially 'turns off' that part of our DNA and stops you from turning into a walking meat vector of whatever it is.
Presumably, whenever it's set to go off naturally, everyone on the planet would spontaneously turn into cancerous meat tumors like you see in the sewers of the game except for the minor few who turn out to have held onto their humanity, presumably having everyone turn into some sort of planetwide meat shoggoth. They had the implications of the infection in the game be WEIRD man. Because presumably all of humanity is a giant ticking time bomb waiting to turn into a pseudo hivemind meat shoggoth for it's entire existence.
