side quest items dont have rarities. if you complete the quest, you get the item guaranteed. they cant be the most rare anything–theyre just something not everyone goes for.
In any bo4 map that has ray gun mark 2 out the box feels like the rarest thing ever. Ive played since release and twice I've gotten it on round one first hit on BOTD. I've gotten pretty damn lucky with it past round 33 again only on blood, the first time I did was right before boss fight and I genuinely did not know it was in the game (other than alpha Omega) blew my mind loved it and made boss fight a breeze lol. Also that was like 4 years ago I haven't seen it since maybe once but didn't need it so I flipped it

I think another set of weapons that most people have never gotten are the time attack reward ones from Gorod and Revelations. Doing the early round time attack is pretty east but the later ones can be rough if you don't have megas or aren't specifically going for them. Another honourable mention would have to be the thundergun on Tag. I swear that and the baby gun are not actually in the box when I play

Ok so the last step you said it wrong. You have to put the scalpel into the tub, bring the warden to the sign and melee kill the warden in front of the sign, return to the tub after the kill to get the sporknife. Strongest weapon in zombies history, as it one hit kills everything after round 100. I used it 5 times🤣😂
