Arthur was basically the ruler at that point, Arthur did all the work, the only work he didn’t do was judge people. If it wasn't because relatives around her respected her decision a conflict like the war of the roses could have happened between the Cromwells and Helena's side of the family is not confirmed whether she had kids or not but thinking about the fact that the royal families are usually refer as "royal houses/noble houses" is because they are extensive and each member has a different role or duty in the kingdom it is almost impossible that there was no more people with the last name or related to Fyher to be claimants to the throne and Melissa, Rachel and Rachel's daughter are also claimants to the throne for what I already explained of Lilith not having children left a open door for a great conflict between Fyhers and Cromwells and chosing Adil was quite unwise because of the implications of a civil war I don't think Melissa or Rachel would dare to declare a war like that but I'm not sure about other members of the house of Fyher I know Lilith did not want to get marry or be a mother but when you take a look of her decision it might have caused a lot of problems to her kingdom in the long term time and probably Adil as king had to face a lot of challenges to his claim to the throne in his already hard life
