You don't have sacrifice the tear, you can use the stone before then he asks if you think that too and then he stay the same way and is happy.

60 hours in and I didn’t know this NPC existed until just now and I refuse to let anything happen to him. If anything happens to him I’m killing everybody. Also are his services different than just altering garments at grace points?

Boc doesn't have a great rune that can provide him a "perfect" rebirth. I think that's the reason of what's happened to him.
