Who is this woman who knows so much of Elias, enough to know where his secret lair is hidden. Seems a little bit suspicious to me, like the woman is either one of his more willing minions or the connection to Andariel is stronger than it at first appears.

I feel kinda sorry for Elias. The guy was probably looking for confrontation with his former comrade, hoping for a prolonged discussion of morality and necessity of evil, maybe even prepared a "Old fool, I'm more powerful than you ever have been" type of monologue…
… and all of that hard preparation interrupted by a shove to the throat. That must be awfully disappointing and unsettling
