The Great Fairy is providing the town Health care. However I don't know if I would go as far to say it's state sanctioned Healthcare, as far as we know the Great Fairy doesn't get paid for her services. Unless you count bringing a Stray Fairy back as payment.
However if the town is paying to upkeep her cave entrance and the pathway, then perhaps that counts as her compensation for providing free health care to Clock Town.
Or maybe it's entirely of on a voluntary basis and she is just providing free healthcare out of the goodness of her heart
