I'm a little disappointed that the Broodmother from Dragon Age: Origins didn't make the list.
Congrats on including Fable. Now, five more, we could easily made it twenty more but these might be a little off the beaten track.
5: Sauron's ending, Primal Rage. We can include any graphic fighter with fatalities and the recent Mortal Kombats, all of it, would qualify for the sheer depravity of the characters. However, for a more concrete example we look to the stop motion claymation dino brawler and how one of the characters is known as the God of Hunger. Basically Sauron must devour the mountains of corpses from the conflict but he will never be sated and must endlessly consume what's left of the human race, until they are wiped out and he starves.
4: The Lonely Island, Mass Effect 2. Jacob's loyalty mission has Miranda let slip his father had been found. Problem was he had supposedly died ten years ago. Should Shepard take Jacob to investigate they slowly discover Ronald Taylor had turned the crash site he was in into Richard Laymon's Island, hunting down and killing the men and keeping the women as sex slaves. The survivors are near zombie like due to the poison food they are forced to eat, and when confronted you have the option of taking Ronald, leaving him the means to atone or just plain leaving him. One of the creepier moments is your third party member squarely addressing him being torn apart, hearing someone like Kasumi sounding like she's looking forward to him being torn apart is disturbing.
3: In Hushed Whispers, Dragon Age Inquisition. Picture the Dreadfort in Game of Thrones. Okay, now imagine it should the White Walkers win. This is the setting of Redcliffe should you go and recruit the mages. The Templar quest is chilling in it's own right but here it turns out the arch mage has allied himself with the demon lord. His attempt to kill you instead sends you a year into the future, where your allies have been brutally tortured and driven mad, the zombie crystals you may have seen about has spread everywhere like it was Tiberium and like the Command & Conquer finance is devouring humans, and the demons have won and conquered the land. Thedas is truly hell and you have to wade through it to get back.
2: The Guard House, Resident Evil 1. Arachnophobes will not have a good time with a number of these entries whether it be Skyrim Dragon Age or Resident Evil. The power station in the third game's remake is basically a nest with a unavoidable encounter and graphic death if you don't react quickly enough, on par with the bloatfly in Revelations. Well entering the pool room of the guard house will have a massive tarantula crawl from the ceiling to the doors you just walked through. We could have the basement in Origins where you have spiders bite you to ribbons, Mortal Kombat X where this is done in first person view, or the shock of this happening in Star Wars Jedi Survivor. But before everyone included spiders in their game the original and remake still delivers on the fright factor even after three Ellen Rose arachnophobe reactions.
1: Death by snu snu, Soul Calibur. Don't laugh. Games are no stranger to luring you in with femimine wiles before killing you. Whether it be the skeleton ghost of Scarlet O'hara ripping you apart in Uninvited, Molly luring you into a trap for Jack the Ripper in Waxworks, a psychotic assassin stabbing you to death in Blood Money or the special ways female zombies can end a survivor in Dead Rising. However, picture a sexually confident girl throwing you to the ground, pinning you in a manner that resembles your fave number, then addressing she's using that large blade of hers to slice your privates off as she does so. You have Seong Mi-na. Or how about a woman who giggles as she places your head between her legs and crushes it with her organs. You have Sophitia. Or how about her sister who jokes about servicing you with doing something very similar to death, she basically has you service her to death. You have Cassandra. Or how about someone who ends the fight with a simple kiss, a kiss that creepily kills you, drains your life, kinda like a vampire. You have the psychotic Tira mounting you. These may not be the likes of the Concentration Camp style environments of Wolfenstein or the vampire assault in Bloodrayne but these women are deadly and, dare we say it, assaultive enough to make some respect the no touching rule.
Haven't reached it yet, but I remember nominating the Man-Bat jump scare from Arkham Knight when you asked. Hope it made it in!
I feel like one of the main reasons behind letting multiple moments from the same game in has to be Resident Evil.
Seeing the first zombie and the first encounter with the dogs both have to be in this list if we're being real.
Sae in Fatal Frame 2 is far more frightening than the falling woman, what a strange choice.
Sorry, but Silent Hill 3 IS the best in the series.
Even after twenty years, the chase with Monster-Ock still horrified me. Doc Ock is a handful. Carnage is terrifying. Ock with the Carnage Symbiote is horrifying. Further amplified by Dee Bradley Baker's phenomenal performance.

Solid list. Although if I were to pick the most unnerving location in Bloodborne (my favorite game), I'd go with Upper Cathedral Ward. The ambient soundtrack alone is enough
