Warning, this is long!
Padme has to die! She died of a broken heart in canon. Bail Organa would be the one to take power and as the friend of Padme, Anakin and Yoda he would have been told what Palpatine really was and what happened to Padme and Anakin. Bail would be sad, but he would not blaim them and say that Padme died because of the Sith. He would keep quiet about the children for now.
Obi-Wan would have to confess that he knew of Anakin's marriage to Padme and blame himself for the fall of the Jedi Order and Ashoka would tell them what Maul had said about Anakin joining the Dark Side and that she had not told anyone and Captain Rex would have to confirm it to be true, and she would be blamed for the death of the Jedi and fall of the Republic and would be soundly punished for it, maybe even killed, on the charge of conspiring with the Sith! No-one would be able to argue against this charge, even Ashoka.
Whilst Yoda and Obi-Wan might be able to restore the Jedi Order, they would have learnt a harsh lesson and realise the Jedi Order failed because they became involved in politics and had betrayed the Force and would have to abandon everything they knew and begin from the old Jedi Order's teaching, or right from scratch. They would have learnt that attachments were not the problem, but having to keep them secret was the problem as it was what pushed Anakin to the Dark Side. They would have learnt that they needed to teach about the Dark Side and not just the Light Side of the Force. They would have learnt that the Jedi need to learn to be peace keepers of the galaxy by following the Will of the Force, not will of the people and let them take care of themselves unless the Force told them to get involved.
The galaxy would need the Jedi at this time, but Yoda and Obi-Wan would have learnt that the Jedi needed themselves first and would focus on themselves first and then go after Maul again to make sure he did not start the Sith Order again! However, if he did, the Jedi would know how to react: work with the New Republic, not as part of it! They would know how to fight the Sith in combat, but would know that the Sith are now in political power, making them stronger than the Jedi! This is another reason the Jedi need to leave politics and go off by themselves and work with the New Republic, but not as part of it like before!
Yoda would likely still have sent Luke to Tatooine to be away from the Jedi and give him a secret protector as a fail safe for if the Jedi Order fell again within his lifetime! Bail would still take Leia so she could take politics and Luke could be the new Jedi Master and work together as brother and sister to deal with the Sith if Yoda failed again!
If Venom can breach into Star Wars. Then I know a perfect one!!! What if Cthulhu and the Old Ones showed during the 1st years of the Clone Wars??

The republic was too big. The Republic should have re-formed around the core and mid rim worlds and make an executive branch separate from the the senate. With such a large government and with a unicameral legislature, the chancellor had too much power and only had to be popular with the elite senators. More independent republic worlds like ryloth that were unhappy with the republic, but also did not like the separatist's should have become independent. The sepratists, after giving up invaded worlds and dissolving the executive council of corporations, should have become legitimized, replacing the council with a president.
Then the smaller republic, independent worlds, and CIS, could join a Galactic Alliance of systems. This focuses on trade and a military defense treaty.
